ABSTRACT

La pérdida de estructura dentaria por debajo del margen gingival y de la cresta ósea alveolar, ya sea por caries, fracturas traumáticas, desgaste, reabsorción radicular o perforaciones iatrogénicas, dificulta el tratamiento protésico y requiere un abordaje multidisciplinario para conseguir resultados óptimos y estables a largo plazo. Se presenta el caso de una paciente adulta que acude a consulta con la pieza 2.3, que no permite una adecuada restauración protésica sin invadir el espacio biológico. Dentro del abanico de posibilidades terapéuticas se selecciona la extrusión dentaria unitaria guiada, para poder exponer un remanente dentario adecuado supragingival, y conseguir un adecuado efecto ferrule para la posterior rehabilitación protésica. La técnica presentada es eficaz, simple, cómoda, higiénica y útil en pacientes que no desean realizarse tratamiento de ortodoncia en ambas arcadas (AU)

ABSTRACT

La extrusión dentaria o erupción forzada es una opción de tratamiento con enormes ventajas cuando nos vemos obligados a tratar fracturas corono-radiculares. Se define como el movimiento en dirección coronal a través de la aplicación de fuerzas ligeras y continuas para provocar cambios en los tejidos blandos y hueso. La extrusión dental forzada amplía las alternativas en cuanto a la rehabilitación futura del paciente, ya que se puede optar por la restauración periférica total con anclaje del resto radicular, o bien por la exodoncia posterior a la extrusión, con la consecuente mejoría de las condiciones del tejido óseo para una posterior rehabilitación implanto-asistida. Presentación de caso clínico, pieza dentaria 2.2 con fractura corono-radicular en la cual se realizó tratamiento de erupción forzada y rehabilitación protésica (AU)


Extrusion or forced eruption es a treatment option with enormous advantages when we are forced to treat crown-root fractures. It is defined as the movement in the coronal direction through the application of light and continuous forces to cause changes in the soft tissues and bone. Forced dental extrusion expands the alternatives regarding the future rehabilitation of the patient, since it is possible to choose either total peripheral restoration with anchoring of the radicular rest of the post-extrusion extraction, with the consequent improvement of bone tissue conditions for subsequent implant-assissted rehabilitation. Presentation of a case report: tooth 2.2 with a crown-root fracture in which forced eruption and prosthetic rehabilitation were performed (AU)

ABSTRACT

The purpose of this study is to identify the factors affecting the treatment outcome after surgical-orthodontic treatment of the maxillary impacted incisors using multiple regression analysis. The study enrolled 83 patients who had surgical-orthodontic treatment in impacted maxillary central incisor between January 2005 and December 2015. Possible explanatory variables related to the prognosis of impacted incisor were age, gender, tooth developmental stage, height, position and angle of the teeth.The results of multiple regression analysis showed that as the height of the stem cell from apical papilla (SCAP) increased, the tooth length ratio increased by 0.345 units (p < 0.01). There was no statistically significant difference in gender, tooth development stage, distance and angle between the center line and the tooth, and the height of incisal tip of the tooth. In conclusion, the height of the SCAP of the impacted central incisor is factor affecting the tooth length after orthodontic traction.

ABSTRACT

Exposing sound structure of a subgingivally fractured tooth using orthodontic extrusion is considered to be a conservative way to re-establish biologic width without sacrificing esthetics or jeopardizing periodontal support of neighboring teeth. When a misaligned tooth is traumatically involved, a more comprehensive approach combining tooth extrusion and re-alignment may be necessary for a successful restorative outcome. This case report describes a successful esthetic management of a patient with complicated crown-root fracture on the maxillary right central incisor and pre-existing malocclusion in the maxillary anterior region. Forced eruption along with re-alignment of teeth by orthodontic movement seems to allow re-positioning of the fracture line to a favorable position and correction of crowding, providing a better esthetic result.

ABSTRACT

Objective:To identify the factors that affect the duration of forced eruption of the impacted maxillary canines.Methods:Data of 46 successfully treated impacted maxillary canines with forced eruption were included.The relation of duration of forced eruption and the age of the patients(y),the distance between the impacted maxillary canine cusp tip and the occlusal plane(d),the angle be-tween the axis of the impacted maxillary canine and the occlusal plane(α),mesiodistal position of the canine cusp tip in relation to the adjacent teeth(ZB),the angle between the canine axis and the lateral incisor axis(β)was analyzed.Results:The results indicated a direct correlation of the duration of forced eruption with the patient age and d value,the more mesial location of the canine cusp tip to the midline,the longer treatment time was needed.In contrast,there was an inverse correlation between the duration of forced eruption and αvalue.Conclusion:The patient's age and the position of the impacted maxillary canines are the important predictors of the dura-tion of the forced eruption.

ABSTRACT

The aim of this study was to report two cases of orthodontic extrusion performed prior to dental implant placement. The first one reports the case of a 57 year-old-female who presented internal root resorption in the left central incisor, and orthodontic extrusion of this element was indicated. The treatment lasted 24 weeks. At the end of this period, the dental implant was placed. The second clinical event refers to a patient of 66 years of age who had vertical fracture in the left central incisor. In this case, the orthodontic extrusion was conducted in 12 weeks. At the end of this period, the dental implant was placed and also the temporary crown, but without occlusal contact. Both cases reported consisted of the involvement of the left central incisor in the aesthetic area and low bone density. The cases reported demonstrated that orthodontic extrusion is a viable alternative to the aesthetic and functional reconstruction with prosthesis over dental implants...


A extrusão ortodôntica é um procedimento clínico utilizado no decorrer dos anos com a finalidade, dentre outras, de preservação e ganho de tecido ósseo para permitir a restauração estética e funcional. Assim, este trabalho teve por objetivo relatar dois casos de extrusão ortodôntica realizada previamente à colocação do implante dentário. O primeiro deles relata o caso de uma paciente de 57 anos de idade que apresentou reabsorção interna no incisivo central esquerdo sendo indicada a sua extrusão ortodôntica. O tratamento durou 24 semanas. Ao final deste período foi colocado o implante no local. O segundo caso clínico refere-se a um paciente de 66 anos de idade que apresentou fratura vertical no incisivo central esquerdo. Optou-se por realizar extrusão ortodôntica, por um período de 12 semanas. Finalizado este período, foi realizada a colocação de implante e provisório sem contato oclusal. Observou-se nestes relatos que ambos os casos apresentados consistiram no comprometimento do incisivo central esquerdo, em área estética e de pouca densidade óssea. Esses dois casos demonstraram a extrusão ortodôntica como uma alternativa viável para a reconstrução estética e funcional com prótese sobre implante...

ABSTRACT

When a tooth adjacent to implant has coronal damages caused by severe dental caries or fracture, the clinical crown lengthening by forced eruption makes it possible to get esthetic restoration due to the prevention of alveolar crestal bone resorption and loss of interdental papilla. A 54-years-old male patient wanted prosthetic treatment because his anterior 3 unit bridges had fallen out. A right maxillary central incisor showed mild dental caries but a right maxillary canine lost most clinical crowns. Forced eruption combined with a gingival fiberotomy of a right maxillary canine was performed for 1 month after the dental implant had been simultaneously placed with bone grafts on a right maxillary lateral incisor. About 5 months after implant placement, 2nd surgical operation was performed. The provisional restorations were adjusted to make esthetic gingival contour for 8 weeks. The porcelain fused gold restorations were fabricated and set. The patient was satisfied with the final restorations in esthetic and functional aspect.

ABSTRACT

This paper outlines the case of a 56 year-old man undertaking treatment by means of luxation and forced eruption of an ankylosed canine. At the time of diagnosis, the ankylosis of the tooth was not suspected, because there were not signs of intrusive luxation nor horizontal diaplacement. Only after the application of a vertical elastic force failed to erupt the maxillary left canine, was the ankylosis of that tooth suspected. At the time of reevaluation, the maxillary left canine hads no physiologic tooth mobility and emitted a sharp, ringing sound upon percussion. Hence, the maxillary left canine was considered ankylosed. The treatment course then changed to the extrusion of the canine through the surgical luxation of the tooth and the prompt application of vertical extrusive forces. The above outcome was successful for the patient not only in the orthodontic aspect, but also in terms of the periodontal considerations.

ABSTRACT

The purpose of this case report is to provide the information of the treatment of the impacted-displaced teeth associated with the disease in the jaw. A 10-year-old boy presented with a large radiolucent lesion accompanying the displacement of the second premolar and first molar in the left mandibular body area. The lesion was diagnosed as unicystic ameloblastoma. As a surgical procedure for the present case, marsupialization was executed. For the eruption of displaced impacted teeth, no orthodontic intervention was done for second premolar while a helical spring was used for the forced eruption of first molar. Good occlusion was established by simple orthodontic intervention.
